Adrenogenital Syndrome in a Girl vs. Micropenis

Left picture: Here, a stage V according to Prader is present in a girl with an adrenogenital syndrome and complete virilization of the external genitals. This diagnosis is only possible with the aid of additional examinations. Suspicious findings are: empty scrotum and non-palpable testicles, a strikingly small and elongated scrotum on traction of the penis in the cranial direction; for that look at the figure at the bottom of the same patient. Right picture: It is a boy with a true micropenis, and not as shown in the additional picture at the bottom of another patient with a seeming micropenis, which has a sagittal skin fold at the base of the penis and is called a webbed penis or a palmure of the penis. In the presented case the micropenis is a part of a severe hypospadias, which is not recognizable in the picture.

Left picture: 3.2-year-old boy with a bilateral empty scrotum; the testicles are neither visible nor palpable. Right picture: 2.3-year-old boy with a bilateral empty scrotum. The penis is smaller than in the contralateral picture and has a minimum length of 1.5 cm without the surplus foreskin on the dorsal side.
